[linux-dvb] scan: no more initial.h

Hi,

I changed DVB/apps/scan to load initial tuning parameters from
a file you name on the command line, instead of relying
on compiled-in stuff from initial.h. That should make it
easier to add new paramter sets in the future, and also
avoids useless tries with wrong parameters.

The file format is simple and hopefully as easy to
understand as it was to implement a parser for it.

If you find errors in the data sets, or wrong or
inaccurate naming, please let me know.
