[linux-dvb] stage 1 of tuner/pll refactoring

This adds the tuner ops API previously discussed on the list and changes all 
DVB drivers to use it. 

In order to keep changes as small as possible, I have done the bare minimum to 
convert things. The next stages will involve cleaning up the drivers to use 
the new API to the fullest extent.

This patch isn't quite ready yet - awaiting confirmation on a few issues with 
cx88.
