While testing with these 2 cards, and rebooting at least 25 times, the 
problem indeed seems to occur only on adapter0, in about 20% of the reboots.

So it seems that your diagnosis is correct, and that this card is in 
fact broken !

I own 2 more of these cards (but they are not here in my office), and as 
I am sure that I have seen the problem on more than 1 card, I am quite 
sure that one of the other 2 must be broken as well, which could explain 
why I missed this. Will further investigate and come back.
