dvb-streams often have "small inperfections" like  packets are lost or brocken, played as pes with timestamp inside the stream its no problem to keep it sync, if you demux the A/V the timestamp is lost and a brocken package will often be ignored so the result is a async stream (audio or video is shorter the the other), beside this it can happen that your PES-stream is so faulty so your remultiplexer simply exits in the middle of the file (lot of programs simply deny the possebility of errors in a mpeg file - makes programming easy)

i´ve never seen something above 10MBit/s but iirc its up to 15MBit/s
