[linux-dvb] Updates to Twinhan DST driver

I've made a number of updates to the Twinhan driver,
including
  - first pass at terrestial card
  - more card types
  - support of type 0x71 cards
  - more PCI ids, can be overridden using card= on bttv driver
  - rework i2c interface so that gpio pin toggling is separate

  still to be done:
   work out exactly what is required for each card for tuning
   packets and CI packets

   bt878 audio dma has to not claim every example of this card
   for itself.

Build and install instructions basically the same except:

   can use card=0x71 if you get very latest bttv driver
   dst automatically includes bt878 module on modprobe

Get it from CVS of dvb-kernel. Be sure to read the README's.
