The VES1820 demodulator found on all cable cards I know (Siemens,
TechnoTrend, both full-featured and budget models) support QAM-128, but I
heard it can be quite hard to get good reception, because QAM-128 requires
excellent line quality...

FWIW, I have had a Siemens-Cable card for a few years, but since it failed
to make me happy, I recently got myself a DVB-Cable Budget card for 99
Euros. Software MPEG-2 decoding eats CPU power, but only about 30% on my
Celeron-1400, and less than 10% on a pentium 4 3,06GHz (measured on Windows
2000 and Windows XP, though;)...
