There you said it, Siemens DVB-C card is known to have more or less
serious problems with QAM128 modulation. There was some talk about this
few weeks back on DVB mailing list, you'll find some hints from there what
to do.

Actually thread was originally about TT / Hauppauge cards but I commented
about Siemens.

Not likely

Try adjusting PWM value, more info
